Getting started

How it works

Am I guaranteed medication?

No. A licensed clinician decides whether treatment is right for you. If it isn't a fit, we explain why, and you aren't charged.

Who can join?

Adults 18 and older whose BMI and health history meet program criteria and who can check in every four weeks. A licensed clinician makes the final decision.

Which states do you serve?

Utah. HZN Wellness treats patients in Utah, and the secure assessment asks for your state.

How long does the review take?

Most assessments are reviewed within 24 hours. If your clinician needs recent labs or a short video visit first, you'll be told exactly what and why before you're charged for it.

What is the Horizon Strong Plan?

A custom diet and workout plan made for you, free with every program and refreshed at your four-week check-ins.

Where does the medication come from?

HZN prescribes compounded semaglutide and tirzepatide, prepared by a licensed compounding pharmacy in Utah and shipped to your door. Compounded medications are not FDA-approved, and the FDA does not review them for safety, effectiveness or quality.

How is the medication taken?

Both are typically injected under the skin once a week. Your clinician usually starts with a low dose and adjusts it at your four-week check-ins.

Which medication should I choose?

Your clinician decides which, if either, is right for you. If you'd rather not pick, choose Help me choose and a licensed clinician recommends one, or neither, from the same secure assessment.

What if I have side effects?

Tell your care team. The most common side effects of GLP-1 medications are stomach-related, like nausea, and your clinician can adjust or pause your dose. If symptoms are severe or getting worse quickly, call 911 or go to the nearest emergency room.

What if I already take a GLP-1 medication?

Our programs aren't for people currently taking another GLP-1 medication, and you shouldn't take two at once. Don't stop or change a current prescription without talking to the clinician who prescribed it.

Why don't you carry retatrutide?

Retatrutide is still an investigational medication and has not received FDA approval. Federal law also prohibits its use in compounded medications, so we cannot legally prescribe or offer it through HZN Wellness at this time. We look forward to offering it once FDA approved and commercially available.

Pricing and billing

See every charge

How much does it cost?

With monthly refills, semaglutide is $69 for the first month, then $159 a month; tirzepatide is $99, then $199 a month. Pay as you go is $159 or $199 every month.

What's included?

Clinician review, medication if prescribed, your free Horizon Strong Plan (a custom diet and workout plan), a check-in every four weeks, and messaging with your care team. Nothing is added on top.

Am I charged if I don't qualify?

No. Your card is charged only after a licensed clinician approves you, then on the same date each month if you choose monthly refills. If treatment isn't right for you, you aren't charged.

Which plan costs less?

Monthly refills save $90 on your first month of semaglutide and $100 on tirzepatide, and you can still cancel anytime, for any reason, with nothing to repay.

How do monthly refills work?

Your plan renews each month at $159 (semaglutide) or $199 (tirzepatide) until you cancel.

Do I need insurance?

No. Programs are self-pay, and we don't bill insurance. HSA and FSA cards are accepted, and your care team can give you a receipt for your records.

What if my dose changes?

If a dose change would change what you're charged, you'll be told before your next charge.

Will I pay for labs or a video visit?

If your clinician needs either before prescribing, you'll be told exactly what and why before you're charged for it.

Cancellation

The fine print

Can I cancel?

Yes, anytime, for any reason, with nothing to repay. To cancel, message your care team in your patient portal before your next refill is processed, and you never owe the months you don't use.

Do I have to stay a set number of months?

No. Cancel anytime, for any reason, with nothing to repay, even after your lower first month.

Can I get a refund?

Medication already prescribed, filled or shipped for the current month can't be refunded, subject to applicable law. Once you cancel, all future billing and refills stop.

Does switching medication count as canceling?

No. If your clinician switches you to the other medication, your plan carries on.

Shipping

How is the medication shipped?

From a licensed compounding pharmacy in Utah to your door.

Do I pay for shipping?

Any shipping charges or taxes are shown before you pay, so the total you see is the total you're charged.

Does this website collect my health information?

No. Your assessment happens in the secure patient portal, and our website analytics stop the moment you leave this site for it.

Who can see my information?

Your care team, and the people who need it to fill and ship your prescription, like the pharmacy.
