You'll finish in the clinic's secure patient portal, on your phone or computer. It's shared with Horizon Spine and Pain of Utah, Dr. Partridge's pain clinic, so you'll see their name there too. Nothing personal is asked on this website.

What happens in the patient portal

Create your account
Enter your email and date of birth. On the next screen, choose HZN Wellness as your location, then type in the code we email you.
Right after
Ask for an assessment
Request an online assessment and choose Help me choose from the list.
Once confirmed
Answer your health questions
Your care team sends you the health questions to finish on your phone or computer. Have your medication list handy.
Usually within a day
A clinician reviews
A licensed clinician reads your answers and decides. You're charged only if you're approved.
